• IMPORTANT 2012-2013 Pre-Kindergarten Round-up Registration and Packet
• Kindergarten Registration Date/Times February 8, 2012 9:00-12:00. This registration is for only those students NOT currently in PreK at Jesse Owens.

NOTE: The student MUST accompany their parent to the registration.

In order to register your child at LeBlanc Elementary School, the following documents are required.

1.  Original State of Louisiana Birth Certificate (Birth Certificate- From New Orleans Vital Statistic)

2.  Child's Social Security Card

3.  Completed /Updated Immunization Record

4.   Proof of Domicile

• Proof of residency, electric bill - Current Utility Bill (must be provided)  If the utility bill is not in the custodial parent's name an affidavit form  provided by  the school must be completed.

• One of the following

• Gas Bill, Proof of ownership (homestead exemption, Rent receipt, Home Owners Insurance)

• These items must be in the name of  the custodial parent.  All documents must show the physical address, not a P. O. Number.

5.  In case of divorce/separation, court appointed custody papers.

6. Transfer form from the last school attended and/or last report card
