Meaux Elementary Registration
Documents Needed
In order to
register your child at Meaux Elementary School, the following
documents are
required.
1.
Original Full Birth Certificate, must have raised State
Department Seal
2.
Social Security Card
3.
Current Immunization Card
4.
Two Proofs of Domicile

Current Utility Bill (must be provided) If the
utility bill is not in the
custodial parent's name an
affidavit form provided by the school must
be completed.

Second Proof of Domicile (water, gas, rent receipt, or
proof of home ownership) These items must be in the name of
the custodial parent.
All documents must show the physical
address, not a P. O. Number.
5. In case
of divorce/separation, court appointed custody papers.
6.
Transfer form from the last school attended and/or last report
card.

Proof of income (for Pre-K only) – Must provide one of the following:
W-2 forms for current income records, OR
Federal income tax
return form 1040 per household member.

IEP, if applicable
