UHC HEALTH ASSESSMENTS

EASY ACCESS HEALTH ASSESSMENT INSTRUCTIONS!!!

"Sometimes the most important step is your first"

• Complete individualized login at myuhc.com

• Click "Health and Wellness" tab

• Click "Complete Health Assessment"

This is a 52 question assessment which provides you with immediate feedback on the current state of your health.  It takes approximately 15 minutes to complete, and your responses are used to help create a customized online experience.

• Identify your personal health needs and "lifestyle score"

• Learn healthy habits

• Access health improvement resources

Your health assessment results are completely confidential as mandated by HIPPA compliance regulations. 

ONLY aggregate reports of wellness needs will be available to those involved in the program.

If you are not insured through the VPSB, you may complete the same Health Assessment through Health A to Z.  Register on the site and complete the assessment.  Access the website thru the link in the left column.

 NON UHC INSURED EMPLOYEES

1) Go to www.healthatoz.com
2) Click on the yellow box located in the left column "Find out your health risks"
3) Now register with Health A to Z...following their requirements for number and
character for username and password.
4) Once you are logged in click on the yellow box again "find out your health
risks..take the health a to z health assessment"
5) Scroll down to "continue to HA"
6) Now click on the purple bar stating "launch university of Michigan Health
Assessment"
7) Now answer the cigarette smoking question and hit "enter"
8) This should bring you to the 52 question assessment.
9) When complete...review the information and submit the assessment.
